First Visit

Three Month Minimum. Monthly Auto Debit. Unused visits don’t roll over.

Two Time Per Week:   Individual: $275   Couple $400   Family $550

Bulk & Per Visit

12 Visit Package ($40 per visit. Expire in 12 months)   $480

Because of the unique nature of this fee system, we cannot, and will not bill traditional health insurance. We can give you an itemized receipt, however it will not include any diagnosis or service codes.
By setting up auto-debit payments and not having to work with insurance companies, we are able to provide lower monthly fees. Unused visit do not roll over. Family memberships are set according to the highest frequency member. Auto-debit memberships will be billed monthly, based on your start date. Memberships are a minimum of three-month commitment. We ask that if you are needing to cancel after this time-period, please email drmichellesummers@yahoo.com (and receive confirmation back) at least 7 days prior to your next billing date.

We do not accept MVA, PI, Workman’s Compensation, or VA disability cases. If you are involved in a car accident or other injury case, your membership will be paused. We will refer you to another provider who will take great care of you. You may return to our wellness practice after you have been released from care and your injury case has been closed
